We should also have clipboards preloaded for various CERT roles, each with an example filled out form for reference.
Please select scribes for their ability to write legibly, preferably printed not cursive.
Unreadable or ambiguous writing is useless. It requires following up - which can be very hard when time or people have moved on and in the worst case can cause teams to be dispatched to wrong locations, endangering people and property while time is lost correcting errors. That is why we pedantically drill people to write legibly on the sign-in form. It starts there.

I am glad you asked 😉 We keep a full set of these forms in our trailers inside organizer pockets in the red bin next to the trailer door up front.
If we need extra forms, we have a printers in the ICP and Staging area.
